Indian citizens require a single entry visa (VDR) to enter Malaysia for employment purposes. This is processed subject to medical clearance and international securiy check.

But your post here states that ISC OR VWR is processed subject to medicals. Is this only applicable for employees less than 5k on?

Yes, as the article says, only Category III (earning under 5k per month) and from countries listed require a medical.

However, your employer may request one if the insurer providing the mandatory corporate medical coverage for them stipulates this requirement

You mentioned not being requested to do a medical "yet" - so I explained the work flow for immigration EP approvals.
