Transfering Social securiy or pension benefits to another EU Country

Hello,

I have been working in Belgium for the last 3 years and currently planning to move to Germany.
I would like to know some details regarding the transferring of social security and pension from Belgium to Germany.

Is any special form I have to obtain from Belgium Administration? Please guide me the procedure if any.

You really need to talk to the social security provider. 3 years it would seem that one has lost it.
