Contact Board of investment of Mauritius, & contact Mauritius Revenue Authority of Mauritius. Type these key words in google search, you will get their websites.

Dear Boris,

I am unsure about the information that has been given to you. It will depend what kind of company you would want to form and for waht purpose. I am in the offshore sector so this is my field of expertise.

Going to BOI or MRA will not help you as these are our institutions whoch govern company. They will not help you for form.
