This is a very specific question that readers of this forum are unlikely to be able to answer.

I think it is best you ask the consulate, or the relevant authority in Germany!

Never dealt with such a thing but Googled the form V0901 (link below). It needs to be filed with the Deutsche Rentenversicherung (German Retirement Insurance office). They are obviously the ones to contact with any such question.

The form is wirtten in both German and English. Section 13 states that any documentation needs to be submitted in its original for OR be certified/notarized by the issueing agency in the corressponding country. Thus an India issued document needs to be notarized in India and a German issued one by the German issuing agent.

As far as the signature, it doesn't mention requiring a witness or notarization of any kind. Maybe best to ask them directly if in doubt.

Thank you for the information, I have applied and got my refund in two weeks. Used V0901 form, get the signature from Regional Public Provident Fund office. Submitted all the documents. If any one wants how to apply or any doubts please ping me, I will help you out. Do not waste money to consultants. It’s very easy and straightforward process.
