if your family is already here and you need to extend their visa then you have to get them medically test at the medical commission. This is one criteria which is required to be completed if you want to extend the stay after 3 months.

No it won't. Confirmed from two staff at the medical commission that they only conduct HIV test and chest x-ray for family visa.
