My experience (memory not 100%) that I wrote to the office Social Security in the Philippines; they helped me find the form online. I filled it out with signature, scanned it,聽 and sent it back to them. Done.

I'm virtually certain that I have answered this question at length, more than once in this forum, including the proper link for filing as well as the phone number, Since the Social Security administration very often requires a conversation with you to make sure that you want to drop your plan b. That's the only thing that you stop. Your normal plan A stays in effect but can only be used when you return to the United States.
