I don't know any doctor specifics, but I think superdoc (dot bg) is a good (and very popular) site for finding various doctors and specialists in your area. You can see their profile/feedback and book an appointment online, and specify private or NHIF. Even if private, an initial consultation is rarely very expensive. They don't always specify if they speak English or Spanish unfortunately, but English is fairly common with docs. And anyone with very good feedback as helpful/considerate doc will likely be OK with doing a bit of phone-based translation, if necessary.

There is a space for notes in your appointment booking, so I suppose it might work if you specify that doc must speak good english or spanish, and please advise so you can cancel appointment if they are not comfortable with such a patient.
