Dependant visa validity

My friends wife is on dependant visa, she left in Jan and her visa validity is till Jul, she is now pregnant. She will not travel until maybe another year. Can my friend extend her visa in Jul to another year, even though she hasn't travelled back to Bahrain, Is there any 6 month rule that person on dependant visa has to travel back to Bahrain otherwise visa will be cancelled?

The visa can be renewed but the person needs to be in Bahrain for the renewal.

Suppose its extended for 1 year aka 12 months when person is here, later she doesn't travel back to Bahrain for 11 months, so during 11 months absence in Bahrain, visa will not lapse right, that the main question.
