Already answered a similar question.  You have to apply to NPRA to extend it.  I think they extend it once or twice, up to a max of another 90 days.  But who knows if this has become relaxed during covid.

i tried extending. but there was no option to extend the stay. there is option to extend visa only.when i tried extending ,they showed 14 days.

I'm in exactly the same situation. Arrived on a 1 year visit visa in December, my first 90 day stay period is up at the end of March. That's the only date that bahrain.bh shows and I can click 'extend visa', which appears to add 14 days to that current date. I haven't clicked confirm on that yet, as I'm hoping that I can get a longer extension somehow (I'm thinking maybe calling NPRA) - ideally I can find ways to extend my stay until the Saudi border opens, so that I can avoid doing a flight-based visa run during COVID.

Me too..I haven't opted for extending the stay for 14 days.

For anyone else in a similar situation: I couldn't find any other options and couldn't get through to NPRA on the phone so I extended online at Bahrain.bh, twice (2 weeks each time) and now I've gone to request a third extension and it's not allowing me to (says to contact NPRA which is effectively a 'no'). So it looks like I need to do a visa run during a pandemic
