DATAFLOW REPORT UNABLE TO VERIFY

I got my dataflow report but the result is unable to verify because there's no respons from my previous workplace. I tried to contact them and they just tell me that they dont use the same email n phone number as in my certificate of employee. I think i wanna request for re-verification.. is there anyone have same problem with me?? What i need to re-verification if the problem like this. And do you think i can get eligibity number with my dataflow report like that??
