I can reply to your first question: you need to visit Paraguay at least once every 3 years for a min of 10 days in order to maintain your residence privileges.
... but honestly I think nobody will ever monitor your staying in the country even if things might always change in the future of course.

you can find the relevant text in the Immigration law. You can't lose the active contact with the country for three years. Practically, it means a visit. However, if you use your Paraguayan resident status for tax planning, then you have to maintain a residential address and at least get a RUC. Now, normally nothing happens if you don't visit Paraguay for three years. However, the new border system shows the date of your last visit and now you can lose easily your permanent resident status. More and more residents are losing their status every year.

This is the relevant part of the law: "5) Cuando injustificadamente permanecieran fuera del
territorio nacional por un lapso superior a tres a帽os, salvo la
pr贸rroga prevista en el Art铆culo 24; y, " (Ley migraciones 978, Capitulo 5, art 34/5)

Hopefully this Covid-19 will be mitigated to an acceptable level and borders have reopened by 4th quarter 2020.聽 But as of June 10th, Paraguay has had very low numbers of cases and fatalities.

I don't think it was in existence when I last visited Asuncion in 2018.

Nothing special, just better-organised information exchange and databases. We have several clients, who searched for our help after losing their permits during the last two years. On the other hand, several foreign governments are also asking for information regarding the tax residence and then it becomes clear that the "resident" does not even visit Paraguay but claims to pay taxes here.
